The Community Support or Case Management services at Renaissance Behavioral Health offers individualized patient care for both substance use and mental health disorders.
Case Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote patient safety, quality of care, and cost-effective outcomes.

Case management services are often necessary for individuals with complex personal backgrounds or clinical histories. Our qualified mental health specialists, coordinate communication between doctors, therapists, counselors, and social workers to ensure that you or your loved ones therapeutic, medical, or social needs are met.
If you need help with mental health or substance abuse, Renaissance Behavioral Health will put you on the right path by completing an initial walk-in assessment. During this session, we will discuss your needs and past medical and mental health history. Our intake specialist will also discuss our available services and recommend next steps, including assigning you to a Case Manager if needed.

Our specialists provide case management services across the greater Cleveland Area, both on an in-person and with minimal sessions provided remotely. We provide behavioral health, addiction case management, and other services for individuals with a variety of concerns and needs.
